HOMEOWNER AND MORTGAGE COMPANY ORDER FORM
THE PRODUCT ORDER FORM CONSIST OF FIVE (5) SECTIONS - THE FORM IS A UNIVERSAL FOR ALL USERS
The Product Order Form consist of five (5) sections requesting various information for Homeowners and Mortgage Lenders. Please fill in what information you have. If you do not have the information that is ok but subject address, contact name and e-mail address is needed for us to start the appraisal order process.

SECTION A - GENERAL INFORMATION FOR ALL USERS
Is General Information for us to use to process your request for appraisal or home inspection products. This section is for both Homeowners and Non-Bank Lender Clients to use alike. While all the information in Section A is not required, most is needed and we do ask that you fill out as much information as possible in Section A in order to assist us in processing your request as quickly as possible.
SECTION B - HOMEOWNERS SECTION
Is Information for Homeowners and Service Providers to complete. We request that Homeowners fill out items that pertain to the request that they are making.
**SPECIAL NOTE: IF ORDER IS BEING PLACED BY ATTORNEY PLEASE ANSWER SEC B-3**
SECTION C - (NON-BANK) - MORTGAGE BANKERS & BROKER LENDERS SECTION
Is for Non-Bank Lenders to complete. We request that Non-Bank Lenders fill out items that Pertain to the request that they are making. If your company is a bank under Federal Guide Lines Please Fax Your Order.
BANKS FAX US FROM THE INTERNET USING A FAXAGE ACCOUNT
SECTION D - PROPERTY INFORMATION SURVEY (REQUESTED BUT NOT REQUIRED)
Is for both Homeowners and Non-Bank Lenders alike and is our Homeowner Property Information Survey. Section D IS NOT required and is available for clients to fill out in order to assist us and our vendors in processing your product request quicker. While this section is not required, we do encourage you to review this section and if possible fill out the items that pertain to your request or information that you may have readily available. Apella Real Estate Business Solutions appreciates your time and assistance in submission of information allowing us to better communicate with our vendors and ease your order process.
SECTION E - EXIT QUESTIONS (OPTIONAL)
Is for both Homowners and Non-Bank Lenders alike and is a few basic and quick exit questions that are NOT required, however are asked so that we may be able to better serve our clients in the future. Apella Real Estate Business Solutions appreciates your time and assistance in submission of information for this Section.

Apella accepts payments via several methods however we do prefer for payments to be made electronically making the transaction smoother for all parties. Please feel free to contact us should you have questions or request.
